- Introduction & Review of Your Intake Form: We’ll take time to get to know you and what your goals are.
- Health Assessment & Personalized Care Planning: we will assess your current condition(s), medications, and any areas of concern (wound care, mobility, chronic conditions, education).
- Service Overview & Consent: We’ll walk you through our services, answer questions, and review the consent forms and agreements.
- Comfort & Dignity First: Every interaction is guided by compassion, respect, and your comfort. We honor your independence and preferences.

What do I need to prepare?

Before our visit, please have:

1. Discharge paperwork or Doctor's Orders
2. Medications/List
3. Recent labs/orders
4. Any supplies needed
5. A clean space for care

 This helps each appointment run smoothly

Do you provide emergency services?

**We are not an emergency service.**

**For emergencies, always call 9-1-1.**

**Nurses are trained in Basic Life Support (CPR, First Aid, and AED)**
