  • Health Assessment & Personalized Care Planning: We’ll assess your current condition(s), medications, and any areas of concern (e.g., wound care, mobility, chronic conditions, education).
  • Service Overview & Consent: We’ll walk you through our services, answer questions, and review the consent forms and agreements.
  • Comfort & Dignity First: Every interaction is guided by compassion, respect, and your comfort. We honor your independence and preferences.

What are concierge services?

Concierge services are private pay care services that you receive in your home or Assisted Living Facility to assist you on your own terms.  

 

What do I need to prepare?

Before our visit, please have:

  1. Discharge paperwork or Doctor's Orders 
  2. Medications/List
  3. Recent labs/orders
  4. Any supplies needed
  5. A clean space for care
This helps each appointment run smoothly




Do you provide emergency services

We are not an emergency service.

For emergencies, always call 9-1-1.

Nurses are trained in Basic Life Support (CPR, First Aid, and AED)
