  • Health Assessment & Personalized Care Planning: we will assess your current condition(s), medications, and any areas of concern (wound care, mobility, chronic conditions, education).
  • Service Overview & Consent: We’ll walk you through our services, answer questions, and review the consent forms and agreements.
  • Comfort & Dignity First: Every interaction is guided by compassion, respect, and your comfort. We honor your independence and preferences.

What do I need to prepare?

Before our visit, please have:

  1. Discharge paperwork or Doctor's Orders 
  2. Medications/List
  3. Recent labs/orders
  4. Any supplies needed
  5. A clean space for care

Do you provide emergency services?

We are not an emergency service.

For emergencies, always call 9-1-1.

Nurses are trained in Basic Life Support (CPR, First Aid, and AED)
